REGISTRATION

  1. Golf registration is limited to 272 players based on a first come, first served basis. Get your application in early!
  2. Deadline for application is June 6, 2003.
  3. Registration for any of the events should be documented by the submission of the registration form with payment by check for the appropriate amount.
  4. To register, you must complete and submit one of the following forms:

blue square Active Duty Military or Government Employee Registration Form

blue square Contractor or Non-Government Registration Form


PAYMENT & FEES

  1. Payment can be made by corporate or personal check.
  2. Write checks payable to: Utah AFA/AEF
  3. Registration is required prior to June 6th.
  4. Cancellations received after June 13th are nonrefundable.
  5. Registration cost for Active Duty Military/Government Employees is $175 plus additional costs for spouse/partner event and golf fees, if any are selected.
  6. Registration cost for Contractor/Non-Government Personnel is $400 plus additional costs for spouse/partner golf fees, if any are selected.

TEAM RULES

  1. In order to reserve a space for your team, each team member must be named. Substitutions can be made as long as it is done by June 13, 2003.
  2. Teams will consist of four (4) players. Each individual team member will also be competing for individual prizes and honors.
  3. A registration application is required for each team member.
  4. Team members will not necessarily play in the same foursome unless specifically requested in the pairing section of this application.
  5. Team winners will be determined based on three (3) lowest best ball scores per hole for the two days' play. Scores will be adjusted by handicaps.

PAIRINGS

  1. The committee reserves the right to adjust Handicaps based on past AFA tournament play.
  2. No pairing changes will be made after June 13, 2003. NO EXCEPTIONS!

  • Hubbard Golf Course ----- 9:30 a.m.
  • Wolf Creek Golf Course -- 9:00 a.m.
Friday June 20
  • Hubbard Golf Course ----- 9:30 a.m.
  • Wolf Creek Golf Course -- 9:00 a.m.
Reminder: Both golf courses are spikeless.
